The deadline for submitting is Friday, 12/16/11 at 5 p.m. This online form must be completed if you need to stay after the halls close at 5 p.m. Thursday, 12/22/11; arrive before the halls open at 8:30 a.m. on Friday, 01/13/12; or to stay the entire winter break. If you are being sponsored by a department or program you need not complete the form but are highly encouraged to check with the department or program to make sure you name is on their list.
